在肿瘤发生发展的复杂过程中,最致命的时刻往往是转移的发生。而赋予肿瘤细胞转移能力的核心生物学过程,正是上皮间质转化(Epithelial-Mesenchymal Transition, EMT)。近年来,随着研究的深入,EMT不再被简单地视为一个"全或无"的过程,而是呈现出一系列复杂的中间状态。

一、什么是EMT?从发育到疾病的动态过渡


上皮间质转化 (Epithelial-Mesenchymal Transition,EMT)是一个进化上高度保守的细胞重编程过程,指的是极化的上皮细胞失去细胞间连接,经历多重生化改变,获得间质细胞特征的过程。在形态上,细胞从鹅卵石样的上皮形态转变为纺锤形的成纤维细胞样形态。这一过程在胚胎发育(I型EMT)、伤口愈合与器官纤维化(II型EMT)以及肿瘤进展(III型EMT)中均发挥着关键作用。

值得注意的是,近年来的研究强调,EMT并非二元转换,而是存在多种中间/杂交态(Hybrid E/M)。这意味着细胞可能同时表达上皮和间质标志物,这种混合状态赋予了肿瘤细胞更强的干性和可塑性,使其更能适应循环系统中的物理应力,进而形成转移灶。

二、EMT的典型特征


EMT的发生伴随着一系列分子标志物的严格转换。这不仅是定义EMT的金标准,也是抗体检测的核心靶点。

1.上皮标志物下调:最显著的特征是E-钙黏蛋白(E-cadherin)的表达缺失。E-cadherin是维持细胞间黏附的关键分子,其表达受到抑制直接导致细胞连接解体。此外,紧密连接蛋白(如Occludin、Claudins)和细胞角蛋白(Cytokeratin)也显著下调。

2.间质标志物上调:细胞开始表达N-钙黏蛋白(N-cadherin)(即“钙黏蛋白转换”)、波形蛋白(Vimentin) 以及α-平滑肌肌动蛋白(α-SMA)。这些蛋白赋予细胞更强的迁移和收缩能力。

3.基质金属蛋白酶(MMP)激活:MMP-2和MMP-9等酶的表达增加,帮助细胞降解基底膜,侵入周围组织。

三、EMT与疾病


肿瘤侵袭与转移:EMT是启动肿瘤转移级联反应的“开关”。通过EMT,癌细胞从原发灶脱离,进入血管或淋巴管(内渗)。在远端器官,这些细胞通过反向的间质上皮转化(MET)重新获得上皮特性,形成转移灶。

治疗抵抗与肿瘤干细胞:发生EMT的肿瘤细胞通常具有肿瘤干细胞(CSC)的特性,表现出对化疗、放疗及靶向治疗的显著耐药性。这种“耐药性”是临床治疗失败的主要原因之一。

器官纤维化:在肾脏、肝脏和肺脏中,炎症或损伤可诱导上皮细胞发生EMT,转化为产生胶原的成纤维细胞,导致细胞外基质过度沉积,引发器官硬化。

肿瘤免疫逃逸:新近研究发现,EMT进程与免疫检查点分子(如PD-L1、B7-H4)的表达呈正相关,从而帮助肿瘤细胞逃避免疫系统的监视。
